CASE
A 25-year-old man presented with neck and upper-limb pain. He underwent surgery for the resection of a left hypoplastic first rib, which was causing thoracic outlet syndrome. The subclavian vein was retracted through an infraclavicular incision, and an endoscope was inserted dorsally to the vein. With the assistance of endoscopy, the first rib was partially resected, and scalene muscles were released from the rib. The preoperative symptoms improved after surgery without any complications.
CONCLUSION
The endoscopic-assisted infraclavicular approach enabled sufficient resection of the hypoplastic first rib, leading to satisfactory outcomes. This method can be considered as an option for resecting a hypoplastic first rib.
病例
一名25岁男性因颈部和上肢疼痛前来就诊。他接受了手术，切除导致胸廓出口综合征的左侧发育不全的第一肋。通过锁骨下切口将锁骨下静脉牵开，在内镜从静脉背侧插入。在内镜辅助下，部分切除第一肋，并从肋骨上松解斜角肌。术后术前症状改善，无任何并发症。
结论
内镜辅助锁骨下入路能够充分切除发育不全的第一肋，取得了满意的效果。该方法可被视为切除发育不全第一肋的一种选择。
