A preganglionic Horner's syndrome developed shortly after ipsilateral percutaneous insertion of a Swan-Ganz catheter in the internal jugular vein for management of pulmonary edema. There were no other neurologic findings nor evidence of mass lesions in the neck or pulmonary apex. This is believed to be the first reported case of Horner's syndrome complicating Swan-Ganz catheterization, which may occur more commonly than is generally appreciated.
