Abstract

PURPOSE

To investigate potential association between different types of frailty and hearing handicap in the older population.

METHODS

A study was conducted on frailty among older adults in the context of social vulnerability. The study involved 229 participants who underwent physical, cognitive, and social frailty assessments. Physical frailty was assessed using Fried's Frailty Phenotype, while cognitive frailty was characterized by the presence of physical frailty and cognitive decline. The Makizako index was used to assess social frailty, and the HHIE-S questionnaire was applied to quantify hearing handicap. Participation restrictions related to hearing difficulties were explored in relation to the three types of frailty using logistic regression.

RESULTS

Hearing handicap were found to be associated with physical, cognitive, and social frailties. However, in a multivariate binary logistic regression analysis, the emotional scale of HHIE-S was only a predictive factor for physical frailty, along with older age, lower education, and the presence of comorbidities. Age and the presence of comorbidities were the only associated explanatory variables for cognitive frailty. Social frailty was only associated with the presence of cognitive changes.

CONCLUSION

Hearing loss-related participation restrictions can be a significant challenge for older adults. Those who also have an emotional impairment, caused by hearing loss, are even more vulnerable to becoming frail or pre-frail. It's important to prioritize the needs of this population and provide the necessary support to enhance their quality of life and prevent further decline.

目的

探讨老年人群中不同类型的衰弱与听力障碍之间的潜在关联。

方法

在社会脆弱性背景下对老年人的衰弱情况进行了一项研究。该研究纳入了229名参与者,他们接受了身体、认知和社会衰弱评估。身体衰弱采用弗里德衰弱表型进行评估,而认知衰弱的特征是存在身体衰弱和认知功能下降。采用牧坂指数评估社会衰弱,并应用HHIE-S问卷对听力障碍进行量化。使用逻辑回归分析探讨与听力困难相关的参与限制与三种衰弱类型之间的关系。

结果

发现听力障碍与身体、认知和社会衰弱有关。然而,在多变量二元逻辑回归分析中,HHIE-S的情感量表仅是身体衰弱的一个预测因素,此外还有年龄较大、教育程度较低和存在合并症。年龄和合并症的存在是认知衰弱仅有的相关解释变量。社会衰弱仅与认知变化的存在有关。

结论

与听力损失相关的参与限制对老年人可能是一项重大挑战。那些因听力损失而存在情感障碍的人甚至更容易变得衰弱或处于衰弱前期。优先考虑这一人群的需求并提供必要的支持以提高他们的生活质量并防止进一步衰退很重要。
