Aorto-caval fistula associated with thoraco-abdominal aneurysm or with multiple segmental aneurysms is a major challenge for the vascular surgeon. We report two patients who were managed successfully. In addition to the classical surgical techniques of aneurysm repair, we used haemodynamic monitoring with a Swan Ganz catheter, an autotransfusion device and interruption of the vena cava, thus avoiding the three main causes of death in such cases, namely, gross haemodynamic disturbances, major bleeding, and pulmonary embolism.
与胸腹主动脉瘤或多发节段性动脉瘤相关的主动脉腔静脉瘘对血管外科医生来说是一项重大挑战。我们报告了两例成功治疗的患者。除了经典的动脉瘤修复手术技术外,我们还使用了Swan Ganz导管进行血流动力学监测、自体输血装置并阻断腔静脉,从而避免了此类病例的三个主要死亡原因,即严重的血流动力学紊乱、大出血和肺栓塞。
