A young man in his early 20s presented with complaints of a sudden onset of painful blurring of vision and active bleeding from his right eye following an injury while fishing. Examination revealed a triangular laceration in the right lower lid and an inferior 10 mm linear conjunctival laceration. Grade 1 relative afferent pupillary defect was noted with restricted extraocular motility. CT imaging revealed a foreign body in the inferior aspect of the right orbit. Under general anaesthesia, the patient underwent globe exploration via inferior conjunctival laceration, and the foreign body was removed, which turned out to be a swordfish bill. Postoperatively, the patient was managed with systemic antibiotics and steroids and showed an improvement concerning vision and ocular motility. This case report highlights the importance of timely imaging and surgical intervention to extract foreign bodies to impact the patient's visual prognosis.
