A technique has been described which allows replacement of endotracheal tubes, while maintaining control of the airway, using a fiberoptic bronchoscope. The technique was successful in 27 of 29 attempts. While this method does not replace elective tracheostomy, it is indicated for critically ill patients requiring urgent replacement of endotracheal tubes in whom anatomic considerations preclude conventional methods.
