Abstract

BACKGROUND

The current study was conducted to assess the effect of probiotic and vitamin D co-supplementation on migraine index (MI), migraine-specific quality of life (MSQoL), and oxidative stress in adults with migraine.

METHODS

This parallel randomized, triple-blinded, placebo-controlled trial was conducted among adult individuals aged 18 to 55 years with a diagnosis of migraine headache according to the International Classification of Headache Disorders-3 (ICHD-3). Patients were randomized to either multispecies probiotic (4.5 × 10 CFU per day) plus vitamin D (50,000 IU every two weeks) or placebo for 12 weeks. MI, MSQoL, blood pressure, anthropometric, and biochemical variables (25-hydroxy vitamin D, nitric oxide (NO), malondialdehyde (MDA), total oxidative status (TOS), total antioxidant capacity (TAC), and catalase (CAT)) were examined at baseline and after 12 weeks of intervention.

RESULTS

In total, 72 patients (36 patients in each group) with migraine headache were included in the study. The mean age of patients was 37.46 ± 0.98 and sixty five out of 72 patients were females. A significant increase in mean serum levels of 25-hydroxy vitamin D was observed in the probiotic plus vitamin D group compared to the placebo group (p < 0.001). A significantly greater reduction in mean MI (-30.11 ± 6.95 vs. -11.97 ± 3.05; p = 0.01) was found in the probiotic plus vitamin D than in the placebo group. In addition, a marginally significant difference was observed between the two groups regarding changes in serum levels of NO (p = 0.05).

CONCLUSION

This trial showed that probiotic plus vitamin D supplementation can favorably improve MI as well as serum levels of NO in adult patients with migraine. Further research on mechanisms through which the supplementation was effective on MI is warranted.

本研究旨在评估补充益生菌和维生素D对偏头痛成年患者的偏头痛指数(MI)、偏头痛特异性生活质量(MSQoL)及氧化应激的影响。

方法

本平行随机、三盲、安慰剂对照试验在18至55岁、根据国际头痛疾病分类第3版(ICHD - 3)诊断为偏头痛的成年个体中进行。患者被随机分为多菌种益生菌组(每日4.5×10 CFU)加维生素D组(每两周50,000 IU)或安慰剂组,为期12周。在基线和干预12周后检查MI、MSQoL、血压、人体测量指标及生化变量(25 - 羟基维生素D、一氧化氮(NO)、丙二醛(MDA)、总氧化状态(TOS)、总抗氧化能力(TAC)和过氧化氢酶(CAT))。

结果

本研究共纳入72例偏头痛患者(每组各36例)。患者的平均年龄为37.46±0.98岁,72例患者中有65例为女性。与安慰剂组相比,益生菌加维生素D组患者的平均血清25 - 羟基维生素D水平显著升高(p < 0.001)。益生菌加维生素D组的平均MI降低幅度显著大于安慰剂组(-30.11±6.95 vs. -11.97±3.05;p = 0.01)。此外,两组在血清NO水平变化方面观察到边缘显著差异(p = 0.05)。

结论

本试验表明,补充益生菌和维生素D可有效改善成年偏头痛患者的MI及血清NO水平。有必要进一步研究补充剂对MI产生疗效的机制。
