A 53-year-old man developed a surgical site infection after undergoing a pylorus-preserving pancreaticoduodenectomy despite receiving preventive antibiotics. The infection was linked to , a rare and multidrug-resistant gram-negative bacterium from the Enterobacterales order, which mainly affects immunocompromised individuals. Identifying the pathogen proved challenging; standard biochemical tests did not provide conclusive results, necessitating the use of the Analytical Profile Index and a specialized biochemical panel for Enterobacterales. This case marks the first documented occurrence of multidrug-resistant causing a surgical site infection in Tanzania. It underscores the rising presence of uncommon pathogens in healthcare settings and emphasizes the need for enhanced diagnostic methods and infection control strategies, especially in resource-limited environments where antibiotic resistance presents significant challenges.
