Glenoid fractures pose challenges in achieving optimal outcomes with conservative treatment, especially when the fracture is large or severely displaced. To address this, various surgical techniques have been developed. The arthroscopic Latarjet has standardized the approach to the anteroinferior region of the coracoid and glenoid rim. This Technical Note describes an adaptation for managing large, displaced anterior glenoid rim fractures. This approach uses a subscapularis split for horizontal screw orientation, enhancing bone purchase and stability, unlike other arthroscopic fixation techniques through the rotator interval that results in oblique screw positioning. The technique allows for arthroscopic control of fracture reduction and provides stable fixation, offering a promising alternative for treating these challenging fractures.
