Invasive stratified mucin-producing carcinoma (ISMC) is a rare type of cervical adenocarcinoma associated with human papillomavirus (HPV). Stratified mucin-producing intraepithelial lesion (SMILE) is an in situ glandular lesion of ISMC. Such lesions have been reported in histology, but cytologic features of ISMC and SMILE are rarely reported in previous cervical cytology. Herein, we report a case of ISMC combined with usual-type endocervical adenocarcinoma (UEA). Cytologically, most cells showed morphological features of UEA cells. Another small portion of the cells was arranged in layers or scattered individually, with rich mucin in the cytoplasm and stained light pink. The nuclei of the cells were crowded and atypical, with hyperchromasia. This study aimed to discuss the cytological features of ISMC or SMILE and emphasize that abnormal mucinous epithelial cells in HPV-positive patients should be carefully observed to avoid misdiagnosis of ISMC or SMILE.
浸润性分层黏液腺癌（ISMC）是一种与人类乳头瘤病毒（HPV）相关的罕见宫颈腺癌类型。分层黏液产生上皮内病变（SMILE）是ISMC的一种原位腺性病变。此类病变在组织学上已有报道，但既往宫颈细胞学中很少报道ISMC和SMILE的细胞学特征。在此，我们报告一例ISMC合并普通型宫颈内膜腺癌（UEA）的病例。在细胞学上，大多数细胞表现出UEA细胞的形态特征。另一小部分细胞呈层状排列或单个散在分布，胞质内有丰富黏液，染淡粉色。细胞核拥挤且异型，核染色质增深。本研究旨在探讨ISMC或SMILE的细胞学特征，并强调应仔细观察HPV阳性患者中异常黏液上皮细胞，以避免ISMC或SMILE的误诊。
