An abscess of the brain is a serious complication in patients with congenital heart malformation. The great majority of cases are limited to cyanotic congenital heart disease and a peak in the age distribution can be seen in childhood. We report on a 40-year-old man with progressive right hemiparesis due to an abscess in the left basal ganglia. Clinical aspects and additional diagnostic assessment are described. By cardiologic examination including transoesophageal cross-sectional echocardiography a bidirectional sinus-venosus defect could be detected. This finding supported the diagnosis of a so-called 'paradoxical' brain abscess.
脑脓肿是先天性心脏畸形患者的一种严重并发症。绝大多数病例仅限于青紫型先天性心脏病,且在儿童期可见年龄分布高峰。我们报告一例40岁男性,因左侧基底节区脓肿出现进行性右侧偏瘫。描述了临床情况及其他诊断评估。通过包括经食管横断面超声心动图在内的心脏检查,发现了双向窦房结缺损。这一发现支持了所谓“反常性”脑脓肿的诊断。
