We report a case of a 69-year-old female patient with bilateral vertebral arteries (VAs) originate from the ipsilateral occipital arteries.
The patient presented to our hospital with chest discomfort and shortness of breath. Computed tomography angiography (CTA) revealed an atypical VA course, where the V1 and V2 segments of both VAs were absent. As a result, the bilateral V3 segments of the VAs originated from the ipsilateral occipital artery. Additionally, there was aplasia of the transverse foramina of the cervical vertebra, absence of transverse foramina at C2 and C3, and aplasia at C1 (left side), C4 and C5.
This represents the first documented case of bilateral vertebral arteries arising from the ipsilateral occipital artery. The surgical implications and embryological basis of this case are also discussed.
我们报告一例69岁女性患者,其双侧椎动脉起源于同侧枕动脉。
该患者因胸部不适和呼吸急促前来我院就诊。计算机断层血管造影(CTA)显示椎动脉走行不典型,双侧椎动脉的V1和V2段缺如。因此,双侧椎动脉的V3段起源于同侧枕动脉。此外,颈椎横突孔发育不全,C2和C3水平横突孔缺如,C1(左侧)、C4和C5水平发育不全。
这是首例双侧椎动脉起源于同侧枕动脉的文献记载病例。本文还讨论了该病例的手术意义及胚胎学基础。
