Vein of Galen aneurysmal malformation (VGAM) is a rare congenital arteriovenous anomaly characterized by abnormal connections between cerebral arteries and the vein of Galen. This case report details the presentation, diagnosis, and management of a 10-year-old boy from Gabon, who exhibited learning difficulties, macro crania, and generalized tonic-clonic seizures. A cerebral CT scan revealed an aneurysmal malformation of the ampulla of Galen with tri ventricular hydrocephalus, necessitating the placement of a peritoneal shunt. Upon referral to our facility, the patient underwent an angioMRI and malformative work-up, followed by an arteriogram confirming the diagnosis. The subsequent endovascular embolization procedure resulted in significant cognitive and radiological improvements, with no further seizures or signs of ischemia. This case underscores the importance of timely diagnosis and intervention in managing Vein of Galen aneurysmal malformation (VGAM), highlighting the efficacy of endovascular therapy.
大脑大静脉动脉瘤样畸形（VGAM）是一种罕见的先天性动静脉畸形，其特征是脑动脉与大脑大静脉之间存在异常连接。本病例报告详细介绍了一名来自加蓬的10岁男孩的临床表现、诊断和治疗情况，该男孩表现出学习困难、巨头畸形和全身性强直阵挛发作。脑部CT扫描显示大脑大静脉壶腹动脉瘤样畸形并伴有三脑室脑积水，需要放置腹腔分流管。转诊至我们机构后，患者接受了血管磁共振成像（angioMRI）和畸形评估检查，随后通过动脉造影确诊。随后的血管内栓塞手术带来了显著的认知和影像学改善，未再出现癫痫发作或缺血迹象。本病例强调了在大脑大静脉动脉瘤样畸形（VGAM）管理中及时诊断和干预的重要性，突出了血管内治疗的有效性。
