Abstract

OBJECTIVE

To explore the current situation of sleep quality and its influencing factors among older adults with cognitive frailty, so as to provide a scientific basis for improving the sleep status of this group.

METHODS

From October 2022 to October 2023, 1,104 participants were recruited via stratified random sampling. Surveys covered basic information. Frailty Phenotype (FP), Chinese Mini-Mental Status(CMMS), Pittsburgh Sleep Quality Index (PSQI), Geriatric Depression Scale-15 (GDS-15), and Self-Rating Anxiety Scale (SAS). Binary logistic regression analyzed sleep disorder factors.

RESULTS

The median PSQI score was 9 (IQR 7-12), with 64.40% having sleep disorders (PSQI ≥ 7). Analysis by χ found that factors such as marital status, place of residence, average monthly household income per capita, smoking status, drinking status, types of chronic diseases, degree of anxiety, participation in social activities, and physical exercise showed significant differences in relation to sleep disorders among older adults with CF (p < 0.05). Further analysis by binary logistic regression indicated that living in rural areas (OR = 0.706, 95% CI: 0.537-0.930, p = 0.013), drinking alcohol (OR = 0.584, 95% CI: 0.382-0.893, p = 0.013), an average monthly household income per capita of less than 2,000 yuan (OR = 1.413, 95% CI: 1.070-1.866, p = 0.015), participating in social activities daily (OR = 0.607, 95% CI: 0.381-0.967, p = 0.036), participating in social activities 1-3 times a week (OR = 0.507, 95% CI: 0.377-0.682, p < 0.001), and the state of anxiety (OR = 2.393, 95% CI: 1.831-3.126, p < 0.001) were all independent factors affecting the sleep quality of older adults with cognitive frailty (p < 0.05).

CONCLUSION

sleep disorders are prevalent among the older adults with CF with a detection rate of 64.40%. Based on this, it is recommended that medical staff formulate stratified intervention strategies according to the above-mentioned independent influencing factors to effectively improve the sleep quality of older adults with CF.

探讨认知衰弱老年人的睡眠质量现状及其影响因素,为改善该群体的睡眠状况提供科学依据。

方法

2022年10月至2023年10月,通过分层随机抽样招募1104名参与者。调查涵盖基本信息、衰弱表型(FP)、中文版简易精神状态检查表(CMMS)、匹兹堡睡眠质量指数(PSQI)、老年抑郁量表15项(GDS-15)和自评焦虑量表(SAS)。采用二元逻辑回归分析睡眠障碍因素。

结果

PSQI得分中位数为9(四分位间距7-12),64.40%的人存在睡眠障碍(PSQI≥7)。χ分析发现,婚姻状况、居住地点、人均家庭月收入、吸烟状况、饮酒状况、慢性病类型、焦虑程度、参与社会活动情况及体育锻炼等因素在认知衰弱老年人的睡眠障碍方面存在显著差异(p<0.05)。二元逻辑回归进一步分析表明,居住在农村地区(OR=0.706,95%置信区间:0.537-0.930,p=0.013)、饮酒(OR=0.584,95%置信区间:0.382-0.893,p=0.013)、人均家庭月收入低于2000元(OR=1.413,95%置信区间:1.070-1.866,p=0.015)、每天参与社会活动(OR=0.607,95%置信区间:0.381-0.967,p=0.036)、每周参与社会活动一至三次(OR=0.507,95%置信区间:0.377-0.682,p<0.001)以及焦虑状态(OR=2.393,95%置信区间:1.831-3.126,p<0.001)均为影响认知衰弱老年人睡眠质量的独立因素(p<0.05)。

结论

认知衰弱老年人中睡眠障碍普遍存在,检出率为64.40%。据此,建议医护人员根据上述独立影响因素制定分层干预策略,以有效改善认知衰弱老年人的睡眠质量。
