To define the prognostic significance of neurological symptoms in the hemolytic-uremic syndrome (HUS), the records of 68 patients with this syndrome were reviewed. Major neurological symptoms (convulsions and/or coma) occurred in 22 patients. Nine of them had dysregulation of breathing as well. An analysis of other symptoms and characteristics of the disease in these 9 patients revealed a rapid progression of the disease, a high incidence of colitis, and a high mortality rate (8 of the 9 patients) despite adequate seizures control, replacement of renal function and assisted ventilation. We conclude that the prognosis of HUS patients with severe neurological symptoms including dysregulation of breathing is poor.
为明确溶血尿毒综合征(HUS)中神经系统症状的预后意义,我们回顾了68例该综合征患者的病历。22例患者出现了主要神经系统症状(惊厥和/或昏迷)。其中9例还伴有呼吸调节异常。对这9例患者的其他症状及疾病特征进行分析发现,尽管惊厥得到了充分控制、肾功能得以替代且进行了辅助通气,但疾病进展迅速、结肠炎发病率高且死亡率高(9例患者中有8例死亡)。我们得出结论,伴有包括呼吸调节异常在内的严重神经系统症状的HUS患者预后较差。
