Five patients with priapism operated by the Grayhack procedure are presented disturbances were explored. Two of these patients having no fibrosis of the cavernous bodies had total impotence 8 months and 14 months after the operation. The cavernosogram revealed a patent shunt. Potency returned after ligation and division of the saphenous vein. In the other cases cavernosogram revealed normal venous return and thrombosis of the shunt. Fibrosis of the cavernous tissues was here the cause of the impotence. In all cases where erection does not occur within 3 months of the Grayhack procedure the shunt should be ligated.
