Bifascicular block is an unusual manifestation of hyperkalemia. A patient who developed right bundle branch block and left anterior hemiblock as the result of elevated serum potassium concentration is reported. After correction of his hyperkalemia, atrial fibrillation with a rapid ventricular response occurred. There was normal ventricular conduction despite this rate-related stress to his conduction system.
