As a result of improper immobilization in extension following phalangeal and metacarpal fractures, rotation and deviation deformities can occur. The functional disturbance and aesthetic deformities can be corrected with an osteotomy through the proximal phalanx of the metacarpal. The technique and results of this correction osteotomy are described. Prevention of these malunions by proper positioning at the time of primary treatment is emphasized.
