The early glomerular lesion of lead poisoning is illustrated in man and rabbits. The abnormality consists of complete epithelial cell foot process fusion and increased epithelial cell cytoplasmic density adjacent to a normal looking basement membrane. The basement membrane, although apparently morphologically undisturbed, is hyperpermeable to plasma proteins. Proteinuria in the early stage of lead poisoning is thus at least partly glomerular in origin.
