Should the current interplay of consumers and providers continue to shape the supply of health care or should the activity of an intellectual elite replace market forces? These questions represent dichotomous points of view toward the health delivery system--each resting on a number of allied assumptions--which must be reconciled if the health care delivery system is to act in the public interest.
