The de-epithelialized 'turn over' flap is a random pattern flap but it is safe by virtue of the rich dermal blood supply. It is a simple one-stage procedure which can be applied to most skin defects and complicated wounds of the leg. This procedure has been carried out in a series of 35 cases and two illustrative cases are presented to show the results. The design of the flap has been discussed in some detail.
