A rare case of hydatid disease involving the iliac bone is reported. The non-specific presenting history, the intraoperative diagnosis, the multiple recurrences and the relentless local spread of the disease are characteristically demonstrated in this patient over a 14-year follow-up period. An unusual form of radical surgery, resection of the ilium, did not succeed in eradicating the disease.
