22 patients with a negative acid stimulation test 6 to 18 months after total vagotomy and pyloroplasty were followed up with repeated secretory tests over 1 to 10 years. The sensitive, selective and harmless stimulation test with 2 desoxy-D-glucose showed a positive answer in one third of the patients after 1 to 4 years. After 5 to 10 years 10 of 12 patients showed an early positive answer. The absolute values of the PAO2DG remain low so that a late risk of recurrence is not to be expected. Possible explanations of the vagal reinnervation are discussed.
