The extracranial intracranial bypass operation is a relatively new procedure in the treatment of cerebrovascular disease. The clinical indications for the surgery are not yet well delineated. It is concluded that a careful prospective randomized study evaluating the clinical indications for surgery and the comparison to available medical management needs to be completed before the true value of this operation is known.
