The treatment of patient with a carotid-cavernous fistula and complete occlusion of the contralateral internal carotid artery is reported. Bilateral superficial temporal-middle cerebral artery anastomoses were performed to create collateral circulation prior to trapping the fistula. The patient made an excellent recovery.
