Members of the Southern California Long-Term Care Council of the National Association of Social Workers were surveyed to determine the adequacy, location, and types of psychosocial information transmitted from acute care hospitals to nursing homes in their area. The findings revealed that the amount and quality of the information transferred were inadequate and that reporting methods were inconsistent. Recommendations for correcting this problem are posited.
