The trans-sternal access to the lung and its root was used in 31 operations of pleuropulmonectomy. The access made it possible to separate without difficulty and almost without blood loss and to treat the elements of the lung root without its preliminary isolation from the adhesion with the thoracic wall. Before the removal of the lung the author proposes to transfuse the remaining blood from the pulmonary vessels into the vein of the patients. This measure helps preserve about 500 ml of the patient's blood. By means of using the trans-sternal access and inverse blood transfusion the total blood loss during the operation was found to be 5--6 times less which in its turn resulted in 2,5--3 times less postoperative lethality.
