67 patients suffering from rectal carcinoma in the early stage underwent transanal extirpation. The mortality rate was 0, 5 and 10 year survival time was much better than after rectal amputation. No significant complications were observed. the most important point is the correct indication. Frequent postoperative rectoscopic control should never be omitted. The transanal extirpation supplements and enlarges the well-known surgical armamentarium against distal rectal cancer.
