The health care record is a highly prized document. It is important to the consumer, vital to the provider, and demanded by a great variety of third parties. In the face of these demands--many sanctioned by statute and regulations--providers are offered guidelines for evaluating their policies and procedures against the background of consumer concerns and their own ethical and professional standards.
