We report the case of a patient who, under spinal anaesthesia proceded by medication with neuroleptic drugs, developed extreme restlessness and mental disorientation. These symptoms were successfully treated with the anticholinesterase physostigmine (tertiary ammonium base). References are reported which show the spectrum of indications for physostigmine.
