This paper reports a patient presenting with obstructive jaundice in whom a percutaneous cholangiogram demonstrated a fistula between the common bile duct and the portal vein, causing portal hypertension. The complications of gallstones are numerous but the formation of a porto-biliary fistula is extremely rare and as a cause of portal hypertension has not been previously reported.
