For the last ten years the closed technique (Combined Approach Tympanoplasty of Intact Canal Wall Tympanoplasty) has provided us with the best method for the treatment of cholesteatoma. Our objective during this discussion is to report on three technical features which produced significant progress in the quality and reliability of the results which we obtained: 1 - Enlargment of the posterior tympanotomy, 2 - Restoration of the attic wall, 3 - Using incus homograft for functional restoration in cases of missing crura of the stapes.
