Columnar secretory cells similar to those of lactating breast are occasionally seen in otherwise quiescent breast lobules from nonpregnant patients. These foci often contain vacuolated, enlarged, pleomorphic, and hobnail cells that may be confused with neoplastic cells. Nuclear features of malignancy are lacking, however. We present a 47-year-old woman who had focal pregnancy-like change near an excised fibroadenoma and review the literature concerning this finding.
