The results of 226 extended surgeries for extensive cancer of the stomach, including 157 gastrectomies, 53 subtotal resections and 16 proximal resections, have been studied. Metastatic spreading into the lymph nodes along the left-side gastric artery was observed in 56.6%, in the hepatoduodenal ligament--25.2%, round the celiac trunk--29.2% and along the upper mesenteric artery--9.3%. Eighteen (7.9%) patients died after operation. The end results were analysed in 90 cases. Five-year survival after extended surgery was 72.7 (stage IIIa), 30.9 (stage IIIb), 33.3 (stage IVa) and 12.5% (stage IVb).
