We report a rare case of thoracic epidural angiolipoma in a 38-year-old female with clinical, radiological, and histological findings including magnetic resonance imaging. Magnetic resonance imaging can provide preoperative diagnosis of spinal angiolipoma, and complete removal can achieve a successful outcome. The possible embryogenetic origin is, however, not proved.
