Clinical orthotopic heart transplantation has been one of the most spectacular medical success stories of the last quarter of the 20th century. This review describes briefly the history of the development of the immunosuppressive protocols that were responsible for this success, together with details of current immunosuppressive regimens, including both advantages and disadvantages of the various approaches used in different heart transplant centres. Special reference is made to the use of monoclonal antibodies and the newly-discovered immunosuppressive reagent FK 506.
