The author reports an unpublished series of 24 operated cases and considers the surgical problems raised by atheromatous stenosis and obstruction of the innominate artery. At present, excellent results may be obtained thanks to: 1) better precision of the indications, excluding patients with neurological complications undergoing aggravation or permanent neurological signs; 2) at present, operations by the cervical route should be performed when the state of the heart and lungs prevents direct surgery by sternotomy, which remains, however, the method of choice.
作者报告了一组24例未发表的手术病例，并探讨了动脉粥样硬化性无名动脉狭窄和阻塞所引发的外科问题。目前，得益于以下几点可获得良好的治疗效果：1）更精准地把握手术适应症，排除有神经并发症加重或存在永久性神经体征的患者；2）目前，当心肺状况不允许通过胸骨切开术进行直接手术时，应采用颈部入路手术，不过胸骨切开术仍是首选方法。
