Computed tomography (CT) accurately identifies the presence of paraoesophageal, upper abdominal and anterior abdominal wall varices in portal hypertension. We present a case of giant pararectal varices and describe the value of dynamic contrast-enhanced CT in reliably establishing the nature of these structures.
