Neuropathic spinal arthropathy is a rare but well-reported sequela of tabes dorsalis. It is now more frequently seen as a complication of spinal cord injury--particularly after spinal fusion. With improved longevity of spinal cord injured patients we are seeing this entity in increased frequency. Clinical and radiographic aspects are described, and a case history is depicted. Classic treatment has been conservative. Based on the changing nature of its etiology and favorable reports from large centers we advocate anterior and posterior fusion and instrumentation of Charcot's Spine.
