Abstract

Sixty-four patients with unilateral atherosclerotic occlusion of the internal carotid artery were studied at the Bakulev Institute for Cardiovascular Surgery with transcranial Doppler (TCD) ultrasonography and angiography before and after extracranial-intracranial arterial bypass (EIAB). TCD assessed accurately the extent of the collateral circulation in 95% of cases. The haemodynamic effectiveness of EIAB depends on the preoperative state of the collateral circulation; the greatest effect was achieved in patients with the lowest levels. EIABs improved the outcome in patients in the present series; hence vascular and neurosurgeons may be persuaded of the benefit of this treatment in patients with unilateral atherosclerotic occlusion of the internal carotid artery.

在巴库列夫心血管外科研究所,对64例单侧颈内动脉粥样硬化闭塞患者在颅外-颅内动脉搭桥术(EIAB)前后进行了经颅多普勒(TCD)超声检查和血管造影。TCD在95%的病例中准确评估了侧支循环的范围。EIAB的血流动力学效果取决于术前侧支循环的状态;在侧支循环水平最低的患者中效果最为显著。本系列研究中,EIAB改善了患者的预后;因此,血管外科医生和神经外科医生可能会相信这种治疗方法对单侧颈内动脉粥样硬化闭塞患者的益处。
