The conversion of a gastrostomy to a gastrojejunostomy may be impeded by the cranial angulation of the gastrostomy. In this report, a sheath was used to redirect the gastrostomy towards the pyloris allowing placement of a gastrojejunal tube system with a weighted end. The sheath allowed for successful placement of gastrojejunal tubes in four children. The weighted end prevented retraction of the jejunal tube into the stomach.
