Community care for elderly people with dementia is a subject of increasing importance. Yet the ability of primary care workers to identify cases is flawed; the benefits of early detection problematic; and the number of identified cases lower in practice than population surveys would suggest. The role of various groups of workers in managing patients with dementia is discussed.
