A serious complication following the insertion of an a-v shunt for dialysis in a 41-year-old man is reported. Three weeks after the operation, arterial and venous circulatory disturbances occurred in the hand, accompanied by cyanosis distally of the shunt and a lymphatic fistula in the operated cubical area, and followed by the symptoms of a compartment syndrome in the forearm with impaired mobility of the hand. In order to restore normal hemodynamic conditions, the shunt was interrupted and the infected area covered by a local flap. Several weeks later a false aneurysm developed and the circulation had to be reconstructed by an extra-anatomic vein graft. At present the hand is functionally fully rehabilitated.
报告了一名41岁男性在植入动静脉分流器进行透析后出现的严重并发症。术后三周,手部出现动静脉循环障碍,分流器远端出现发绀,手术区域出现淋巴瘘,随后前臂出现骨筋膜室综合征症状,手部活动受限。为恢复正常血流动力学状况,中断了分流器,并采用局部皮瓣覆盖感染区域。几周后出现假性动脉瘤,不得不通过解剖外静脉移植重建循环。目前手部功能已完全康复。
