Researchers in many fields have become increasingly aware of the observer as an important source of measurement error. Consequently, reliability studies must be conducted in experimental or survey situations to assess the level of observer variability in the measurement procedures to be used in data acquisition. The present investigation demonstrated that interobserver and intraobserver variability is generally high, even between investigators internationally recognized for their experience in lymphomatous disorders and even when dealing with noninitial CTCL lesions. Similarly, sensitivity and specificity are generally low, the former astonishingly, without significant differences between early and noninitial CTCL lesions.
