The posterior approach to the peroneal artery is described and illustrated by two case reports. The arterial inflow source was the popliteal artery, and the lesser saphenous vein in the reversed position was the bypass conduit. The approach was found to afford excellent exposure and is the logical choice when the inflow source, the distal target vessel, and the bypass conduit are readily accessible from the posterior aspect.
