Small leiomyomas of the adrenal gland are pathologic curiosities, usually discovered incidentally at autopsy, and thought to arise from smooth muscle of the adrenal vasculature. We report the clinical, radiologic and pathologic features of a symptomatic adrenal leiomyoma, which, to our knowledge, represents the first recorded example.
