Most commonly, sarcoid involves the lungs and thoracic lymph nodes, liver, skin and eyes. The heart, gastrointestinal tract, kidneys and brain may also be involved. The classic form of lymphadenopathy involves both hilar areas and the mediastinum, producing the "1, 2, 3 sign." Adenopathy generally precedes parenchymal pulmonary involvement, which may take several forms.
