An allergic contact dermatitis (ACD) reaction associated with the use of an orthodontic headgear is described in a patient with a history of atopy. The problems relating to the differential diagnosis between these two conditions are discussed. This case highlights that during medical history taking specific questioning relating to nickel sensitivity should be undertaken to identify those patients who may be at risk.
