We describe a very rare case of distal anterior choroidal artery aneurysm. A 34-year-old woman was admitted to our hospital because of headache and vomiting. Computed tomographic scans revealed a diffuse intraventricular hematoma and a thin subarachnoid hemorrhage. Cerebral angiography disclosed a saccular aneurysm in the peripheral portion of the right anterior choroidal artery, which was located in the temporal horn of the right lateral ventricle. We approached the temporal horn through a small inferior temporal incision, and the aneurysm was resected without any neurological damage. A histological examination of the surgical specimen revealed it to be a true aneurysm.
