The nursing team in a 28-bed rehabilitation ward for older people at South Cleveland Hospital recognised a need to improve communications between patients, carers, relatives and members of the medical team. They developed a model for a patients, families and carers clinic to address these issues. This was then tested in the ward over a three-month period, a pilot that continued after the ward had been modified to a 20-bed stroke unit.
